It can be getting into fights, have a illness causing hair to be missing, going through a tough places like under a gate where they can get cut.
Just because he is fixed doesn't mean that other unfixed Tom cats aren't picking on him. He is likely getting beat up by neighbor cats. This can be dangerous and is a leading cause for feline aids which is terminal. If you allow him outside make sure you have him vaccinated. It also could be from neighborhood dogs?
Hey people don't hate. It's okay if your cat is and outdoor cat they seem to be happier that way anyways. I would agree that yo should make sure your cat has all his/her shots just to keep your kitty healthy! It's probably a cat fight they won't kill eachother but you should watch her one day and follow her around to see what she is up too. (:
do your neighbors have other cats ??? i know my two cats arent friendly with the neoghbors cat but they dont come home with scratches ..... are they left out at night? other animals could have gotten to him ..... i feel bad!! !: (
